Your complaints of chest pain and edema make me think of potential heart involvement. Have you considered going to the ER? Also, call your rheumy’s office and ask to speak to one of the nurse’s. Tell her what is going on and emphasize the edema. Try sleeping in a sitting or semi-sitting position. Do you feel shor of breath? If so, get to the ER right away.
