Plaquenil isn't working, Doc suggests Methotrexate

Question:

Hi Dave, I have been taking 15 mg a week of Methotrexate since February in addition to Plaquinil, Imuran, and Prednisone.  There is a danger of infection while taking Methotrexate, so my rheumy put me on Bactrim (which I know a lot of people on this ng can’t handle because its a sulfa drug) 3 times a week. Also, some people have problems with their stomach (nausea, vomiting, pain), but that can sometimes be alleviated with folic acid (I take 2 mg a day).  I only recently began to have stomach problems, but my rheumy suggested that I take half a dose in the morning and the other half at night, which helped a lot.  My stomach didn’t bother me at all this week. Another thing with Methotrexate is that your mom will have to have her liver function tested regularly.  She shouldn’t drink a lot, one thing that I really didn’t have a problem with because I usually only have at most 3 drinks a month. I have no idea if the other drugs I am taking are helping.  I think that in some way I actually need all of the meds I take, but I have no way of knowing if the individual meds are helping. I hope this helps a little.  Good luck to you and your mom. Mel

Response:

It took me like 4 doses (I think) before I did not get sick about 3 or 4 am on Thursday am. I take it on wed nights. I get really tired too. "crazyred" <infnd…@bellsouth.net> wrote in message

Oh yeah, and I lose a layer of skin in my mouth, but folic acid seems to help with blisters. I dunno why, but my skin in my mouth peels. It did not start until I started taking the metho. "Dave D." <durgari…@home.com> wrote in message

Hi my name is fiona I have had lupus for at least 5 years. I have been on Plaquenil for 4 and prednisone on and off for 4 years. As well as NSAIDS. The prednisone has no effect on me any more and I have been on Methotrexate for about 2 months. The first few weeks I had a lot of nausea, which my doctor gave me Ondansetron for . The Ondansetron worked well and I have found I need less and less of it now. I have to admit that the Methotrexate is a horrible drug but I feel better than I have done for years. Nothing ever helped with my fatigue until I took the methotrexate. I guess thing work differently for everyone but it is all a bit of trial and error (like a lot of things with lupus). Good luck to you and your mum and I hope she feels better soon.

Dave, I have only heard about Methotrexate.   I know some say that it can cause severe vomiting until the body gets used to it.  But, if the Dr. thinks she needs it, then I think she should follow orders. BE WARNED about any alternative type meds.  They can have severe reactions when mixed with some rx meds and can kill.  It would be best to get all the ingredients and take them to the Dr. and let him see them first.  He will tell you if he thinks it would be a waste of money or dangerous.  But, unless he knows what is in the product, he cannot help. I hope your mom gets well very soon.  She and you will be in my prayers. HUGS~ Jenn/NC "Dave D." <durgari…@home.com> wrote in message

My daughter has been on Methotrexate (MTX) with fairly good results.  All the DMARDs can have some nasty side effects, it’s true. However, we’ve not had too many problems with the Methotrexate overall, and it has very clearly helped. The main side effect which has been a problem has been GI effects (nausea, stomach pain and occasional vomiting) the first couple of days after a dose. For the most part, we’ve counteracted these side effects by increasing my daughter’s daily folic acid dose.  She now takes 3 mg./day.  (Methotrexate depletes folic acid, BTW.)  Other than that, no serious problems.   Granted that every person is different.  Some people will have more problems with MTX than others.  However, MTX has been used for many years to treat lupus and RA patients, and doctors are very experienced with the side effects and use.   If your doctor is wanting to try your mom on it, I personally wouldn’t see the problem with that.  It might indeed be very helpful.   If it doesn’t help, then there are other possibilities as well.  My daughter’s doctor considered Imuran to be basically equal to MTX.  My daughter, however, wanted to try MTX because of the once a week dosing rather than every day.   Sandra

Hello all,      This is David again.  Well, my Mom’s been on Plaquenil for the past 9 months with no success.  Now the Doc wants to step things up with a stronger med called Methotrexate.  I’ve been reading about it tonight, and it sounds pretty nasty.  Do any of you have experience with this med?  Are the side effects as bad as they sound???  I also found a web page while surfing for info on Methotrexate, and found this.  http://cmo-usa.com/index.html  Some all natural alternative med to Methotrexate called CMO II.  If anybody has info on this stuff, I would greatly appreciate it. Sincerely, David durgari…@home.com
